What is DOT physical?

A Department of Transportation (DOT) physical is a medical exam that ensures commercial vehicle drivers are fit to drive safely. The exam is required for anyone who drives a commercial motor vehicle (CMV), such as a bus or semi-truck, across state lines. Some states may also require a DOT physical for drivers who operate a CMV within state boundaries. The exam is conducted by a licensed medical examiner.

The exam includes:

  • Vision: Drivers must be able to see at least 20/40 in each eye
  • Hearing: Drivers must be able to hear a “forced whisper” from at least 5 feet away
  • Blood pressure and pulse rate: Doctors check for high blood pressure or an irregular heartbeat
  • Urinalysis: Checks for glucose levels and drugs
  • Self-reporting: Drivers provide information about their general health history and existing medical conditions

The exam is typically valid for two years, but may need to be taken more frequently depending on a driver’s medical history. Drivers who pass the exam receive a medical examiner’s certificate, also known as a medical card.
